Consulting
More memories, less worry.
Let us help.
Care Consulting and Transition Management
We’ll spend time getting to know you and your aging loved one, outline options for care and support and create a plan that meets your needs now and later. We will evaluate factors such as needed care, financial considerations and lifestyle so you can make informed decisions that support aging safely, aging confidently and aging well.
If your plan includes transitioning to a senior living community, we will provide guidance on questions to ask, criteria to consider, and comprehensive assistance to ensure a seamless transition.
Navigating a move later in life takes care, compassion, and specialized knowledge. We understand the unique needs of older adults and their families – whether downsizing, transitioning to assisted living, or helping a loved one sell a longtime home, we provide patient guidance, trusted resources, and a smooth, stress-free process every step of the way.
Also, as a REALTOR® with Berkshire Hathaway HomeServices New England Properties, we can help you sell your home or buy a new home in CT. Learn more here.
Care Coordination
Financial and Legal
Ensure your affairs are in order by prioritizing your personal financial and legal matters. We'll connect you with professionals to guide you through choosing executors and beneficiaries, when it’s time to initiate Power of Attorney privileges and ensure protective coverage for you and your family.
Social and Emotional
Engagement and interaction are key components of aging well, wherever you live. We’ll help find activities or companions to improve overall social and emotional well-being, at your comfort level.
Aging in place
If your goals include aging in place, we’ll work with you to conduct a home safety evaluation, coordinate any improvements or repairs and install key adaptive technology – all designed to help maintain independence and comfort.
Additional Support
Meal delivery, companion care, transportation and caregiver support can all be arranged through our care coordination services.
Health
During the chaos of an unexpected illness or injury resulting in a hospital stay, we’ll help you in communicating with the care provider, navigate the next steps of appropriate care and understand the financial implications of that care, and follow through with a comprehensive strategy for continued support.
Educating
Leading the way in educating the next generation of elder care workforce, caregivers and supportive resources.
